National Infertility Day Health Professional of the Year
Awards 2010
The National Infertility Day Health Professional of the Year
Award is presented bi-annually to someone who has shown understanding
and support over and above the call of duty
The winner will be announced at National Infertility Day
and nominations need to be received by 15th May. The nominations
will be considered by the Organising Committee.
The nominee should be someone who you feel has given you
outstanding support and empathy and whom you feel should be
recognised for this. We are looking for someone who has shownempathy
and understanding of the emotional effects of infertility
and who have contributed substantially to helping you through
what may have been a very difficult time.
Entries should state the name of who you would like to nominate,
the address of where they can be contacted and their position.
We also need your name and address and why you want to nominate
them.
